Children being registered at Summit Heights require the following:
- Birth certificate or Notice of Birth (proof of age)
- Passport or Landed Immigrant Status Form for children born outside of Canada
- Ontario Health Card
- Immunization Records
- Proof of residence in Toronto (must be a legal document) – Please check the TDSB website to ensure that Summit Heights is your home school. For the 2014/2015 school year Summit Heights is closed to optional attendance.
- Change of name documents if applicable
- Custody documents if applicable
